We are happy to register new NHS patients within our practice boundary area.
How to Register
To register with our practice please follow the link below to complete the online registration form through the NHS portal.
Please DO NOT COME INTO THE SURGERY at this present time.
To assist with the accurate processing of your registration, you are welcome to send us separately via email one form of identification to nhsnwl.hammersmithsurgery@nhs.net ; if you are registering a child, please submit a copy of the full birth certificate, however this is not mandatory.
Acceptable identification includes:
- Passport
- Biometric residence permits (BRPs)
- Birth Certificate
- Driving licence
- HC2 Certificate
- Rough sleepers’ identity badge
- Hostel Registration / mail forwarding letter.
A member of our admin team will contact you via email if there are any issues with your registration. We aim to complete registrations within 48 working hours. However, at busy periods it may take up to one week.
Out of Practice Boundary Area Registration
Where it is clinically appropriate and practical to register, we now accept new registrations from patients who work in the local area but reside outside our registration area. Patients registered this way would not be entitled to a home visit from the practice. However, if you live outside the practice area and need a home visit, please contact NHS 111 in order to be seen by a practice closer to where they live.
