How to Make an Appointment
Our appointment system is changing. From Monday 15th May, we are asking patients or their representative to request all Doctors’ appointments via our PATCHS system via the link: https://patchs.ai/practice/hammersmithsurgery. If you do not have internet access a member of the reception team will be able to assist. You or your representative can telephone or come into the surgery for further assistance. All other appointments can also be booked via PATCHs or in the usual way
